Medical Privacy and Disclosure Policies and Procedure

Existing and New Medical Conditions

It is the responsibility of the client to keep the therapist informed of any medical treatment or prescriptions he/she is currently undergoing and to provide written permission from a physician, chiropractor, physical therapist, etc., that massage may be received.

Privacy

The privacy of your personal information is important to our clinic. We are committed to collecting, using and disclosing personal information responsibly and only to the extent necessary for the services we provide. Like all medical professionals, we collect, use and disclose personal information in order to serve our clients. The primary purpose for collecting personal information is to provide treatment. We also use personal information to process credit card payments.

We understand the importance of protecting personal information. For that reason, we have taken the following steps: Client records are maintained in a secure online database. Electronic hardware is either under supervision or secure in a restricted area at all times. Paper information is transmitted through sealed, addressed envelopes. Electronic information is transmitted through secure networks. You have the right to see what personal information we hold about you and to request copies. We will try to help you understand any information you do not understand, e.g., medical terminology, etc. We reserve the right to charge a nominal fee for such requests.

Financial Policy and Procedures

Refunds

There is a no-refund policy. Once an appointment is booked it must be completed or will be forfeited. Under special circumstances a gift certificate credit may be issued. However if client is at fault there will be a cancelation fee from 50% – 100% deducted payment. This is a case by case basis.


Memberships are non-transferable to another party.

 

PROMO CODES must be used at online purchase. There will not be a price adjustment after online purchase of service. Please make sure before finalizing massage you apply your promo code. If coding does not fit policy of code it will be removed. 

Membership Policy

Neo Radix Soul Healing Therapy reserves the right to update, change, or discontinue memberships at any time. We also reserves the right to terminate memberships at any time due to failed payments or inappropriate behavior by you or a secondary member. Excluding termination for inappropriate conduct, all unused massages will remain on your account for you to redeem or they may be converted into gift certificates of equal value. Benefits have no cash value and no refunds will be issued for unused benefits.

Gift Certificates

Neo Radix Soul  Healing Therapy  is not responsible for lost or stolen gift certificates. Gift certificates cannot be refunded or redeemed for cash. Gift certificates have no cash value. Gift certificates may not be used to purchase other gift certificates. Neo Radix Soul Healing Therapy reserves the right to refuse service or reverse any gift certificate purchase for any reason and return the funds to the purchaser at the original purchase price. If the service price exceeds the amount of the gift certificate, the redeemer is responsible for the remaining balance. Gift certificates cannot be combined with ANY additional discounts, including: discounted gift certificates, coupons, membership discounts or special offers.

Insurance Billing

Our clinic does not offer direct billing to insurance companies. We can provide an itemized receipt for your session that you may submit to your insurance company for possible reimbursement. You must contact your health insurance provider to verify that services are a covered plan benefit and for instructions on how to submit your claim. You are responsible for full payment at time of service.


Stripe POS Billing

For your security we use one of the top brands for financial protection. This is a safe and secure platform. After your first invoice is sent and paid for financial information is safely secured on our POS platform. This helps with subscription management as well as booking in person for your next service appointment. This information will only be used under the verbal or written agreement of services or tips.

Procedures 

Booking Policy

All clients must book online. Even last minute availability clients must book online and agree to sign forms as well as describe medical concerns and issues before appointment as well as pre-pay for service. There are no exceptions to this rule.

Cancelation Policy

We respectfully ask that you provide us with a 78 hour notice of any schedule changes or cancellation requests. Please understand that when you cancel or miss your appointment without providing a 78 hour notice we are often unable to fill that appointment time. This is an inconvenience to your therapist and also means our other clients miss the chance to receive services they need. For this reason, you will be charged 100% of the service fee for each session after that. We also reserve the right to require a credit card number to be given to book future appointments so that appropriate fees may be charged if a late cancellation does occur. We understand that emergencies can arise and illnesses do occur at inopportune times. If you have a fever, a known infection, or have experienced vomiting or diarrhea within 24 hours prior to your appointment time, we request that you cancel your session. Inclement weather may also result in the need for late cancellations. We will do our best to give advanced notice if we are closing or need to cancel due to bad weather and we ask you to do the same. Please do not risk your own safety trying to make your appointment. Late cancellation due to emergency, illness, or inclement weather will generally not result in any missed session charges, but this is determined on a case-by-case basis.

Late Arrival Policy

We request that you arrive 15 minutes prior to your appointment time to allow time to fill out any required paperwork as well as answer any intake questions your therapist may have. We understand that issues can arise that may cause you to be late for your appointment. However, we ask that you call to inform us if this ever occurs so we can do our best to accommodate you. Appointment times are reserved for each client, so oftentimes we cannot exceed that reserved time without making the next client late. For this reason, arriving after your appointment time may result in loss of time from your massage so that your session ends at the scheduled time. Full service fees will be charged even when sessions are shortened due to late arrival. In return we will do our best to be on time, and if we are unable to do so we will add time to your session to make up for our late arrival or adjust the service charge accordingly.

Draping Policy

First and foremost, draping is to protect the client’s privacy.   Throughout the massage you should feel covered, warm and comfortable and  never work under the sheets.
With leg draping, the sheet is tucked under your leg to completely cover your private areas.  Some therapists tuck the sheet under the same leg and others prefer tucking under the opposite leg. When it is time for you to turn over, your therapist will lift up the sheet and blanket enough for you to roll over while remaining fully covered. Once the massage has completed, your therapist will cover you back up completely before leaving the room.  Once you are ready (take your time getting up as you might be groggy!) you can get up and get dressed. If client refuses draping client must be fully clothed in relaxed loose fitting clothing. However areas that are clothes will not be massaged. Compression work and stretching will be done for those areas instead. 

Behavior and Conduct Policy

Neo Radix Soul prides itself on respect for others and ourselves as well as genuinely building a community and culture with our clients. There will be 0 tolerance for any behavior that is outside of the nature between client and therapist or coach. We honor our clients for their support however we must protect each other with boundaries and be examples of growth in nature as we nurture those who believe in what we represent. The mutual respect between client and therapist needs to be respected or services will not be continued. This is and 0 tolerance policy. Neo Radix Soul services are for elevation and for relaxation towards medically inclined therapeutic purposes only. There is absolutely no sexual, aggressive, or discriminatory,  component in our business whatsoever. Any insinuation, intimidation, joke, gesture, conversation, or request otherwise will result in immediate termination of your session and a refusal of any and all services in the future. Complimenting therapist on work during massage is not necessary nor is unnecessary sounds or overly excessive breathing that cause therapist to be uncomfortable during massage. It is recommended that you rest and enjoy the massage and only communicate discomfort. If you have any issues with self control such as verbal or physical spasms of any sort that represent this behavior even touching the therapist will conclude to the termination of your massage. You will be charged the full service fee regardless of the length of your session. Client has the right to refuse service if he/she feels sexual impropriety on the part of the massage therapist. Depending on the behavior exhibited we may also file a report with the local authorities if necessary. Treat your therapist with respect and dignity and you will be treated the same in return. We are here to build and grow we understand self discovery comes with moments of immaturity or a loss in self awareness to distract ourselves from our burdens but treat those as you would want yourself and loved ones to be treated if they were in the human service field. In order for us to grow mind, body and soul this structure of respect must be the foundation of our business for a better path of living.
